Referring a New Patient

If you wish to refer a patient to Dr. French, please fill out a referral form for our office and fax it to (209) 740 - 4717.

Allow up to one week for our staff to review routine referrals, we will contact the patient to schedule an appointment. Label all EMERGENT and URGENT referrals clearly so that patients can be seen in a timely manner by Dr. French. Thank you!
